Governance rules
- Separation of duties: an author cannot approve their own content or claims.
- Sanctions, legal and regulatory interpretation always require a reviewer approval.
- At least one active admin must exist; admins cannot demote themselves.
- Every role change, invite, deactivation and approval is written to the audit log with a reason.
